This document is password protected. Refer note below for details of password.
Your password is a combination of the first four letters of your name and date of birth in dd/mm format. For example:
- If your name is Amit Kumar and date of birth is 23-12-1956, then your password will be amit2312.
- If your name is M. M. Ramaswamy and date of birth is 17-6-1977, then your password will be mmra1706.
To view the Insurance Certificate, you should have Adobe Reader version 6.0 or above (or a similar viewer) installed on your personal computer or mobile device.
